Psychiatric Rehabilitation

Through our Psychiatric Rehabilitation Program, CCMH offers a wide variety of services for individuals who live with severe and persistent mental illnesses through an evidence-base program to support illness management & recovery, case management, medication management, respite services, and housing options. The Psychiatric Rehabilitation department works hard to help individuals move towards their goals of community-based living, employment, education and self-advancement.

 
 
 

The Psychiatric Rehabilitation Program serves adults with severe and persistent mental illnesses in an evidence-based program including:

  • Assertive Community Treatment (ACT)
  • Illness Management & Recovery
  • Case Management
  • Medication Management
  • Respite Services

We believe that in order for recovery to take place, the culture of mental services must be based on:

  • Self-determination
  • Relationship empowerment
  • Full participation of consumers in work and community life
  • An active involvement in eliminating stigma and discrimination  

With this in mind, the Psychiatric Rehabilitation Department offers a wide variety of services specifically designed to support individuals with serious mental illness move toward their goals of community-based living, employment, education and self advancement.


Housing Options

  • Adult Foster Care
  • Supported Housing Apartments
  • Assistance Locating Independent Housing

Supported Housing: All apartments have 2 bedroom units where residents are paired to enhance the skills of one another. On-site managers continue to assist residents in skill development to gain their final independence level, community living.

Our House: Foster care home that gives additional education, support, and skill development to assist people to their next level of independence under the supervision of an on-site manager.

Cornerstone & Alternatives have continuous staffing where our goal is to assist residents in gaining the highest level of independence for their willingness and capabilities.
